Customized care begins from your first phone call to Silver Tree Home Care. We gather as much information as possible on your needs and wants. We patiently answer any questions you may have and then schedule an in-home assessment.
Care plans are as unique as our clients. The in-home visit enables us to meet with you, your family, and the person who will be receiving care. We’ll gather detailed information to create a custom plan of care for your loved one. We ensure services recommended are carefully tailored to your loved one’s needs. The plan of care is reviewed and modifications are made as you see necessary. We establish a safe environment for clients and their families. Additional attention is dedicated to planning to provide the best care at a reasonable cost.
Next we begin searching for the special person to provide that care. We carefully match personality and services needed to a trained, experienced, reliable, and compassionate caregiver. If for any reason you chose to request a different caregiver, we will quickly find a replacement.
Once care begins, our care manager stays in touch, making home visits and routine follow-ups by phone to ensure client satisfaction.
